This video offers step-by-step instructions on filling out your pre-visit intial or interim history forms and "checking-in" in advance for your appointment.  All payments are currently taken only over the phone before each appointment as of now.  The option to self-pay online is coming in November 2022.

 

You must check in at least 7 days before your first appointment to allow for your information to be reviewed and prepared in the chart.
