Our patient portal, On Patient, serves as a convenient hub for communication and access to essential resources. Here, you can securely update your account information such as demographics, insurance, pharmacy, and payment information. You can also access the portal 24/7 to review appointments and messages, share updates, and access educational materials.

Sign consent forms

Sign all consent forms by 12pm the day before your visit or 12pm by Friday for Saturday, Sunday, and Monday appointments. We are not able to proceed with treatment without them. Late submissions may result in your appointment being cancelled and your time slot will be offered to patients on our urgent appointment waitlist.

Tip: You can conveniently complete the forms on your mobile device. Android users should access the form on a desktop to avoid submission problems.

Complete intake forms

Complete the intake forms by 12pm the day before your visit or 12pm by Friday for Saturday, Sunday, and Monday appointments. Providers review these in advance, saving you time by not repeating everything and also making your initial consultation more fruitful by focusing on the most key problem areas.. Late submissions may result in your appointment being cancelled and your time slot will be offered to patients on our urgent appointment waitlist.

Confirm you received a confirmation email with an On Patient portal invite. If you don’t see the invite in your inbox, be sure to check your spam folder. Note:

Access the confirmation email you received at the time of scheduling or the most recent appointment reminder email you received in your inbox.. Click on the On Patient registration invitation. You will verify and complete your account information including the most up-to-date insurance policy, allergies, pharmacy, and payment information. You must click the green "I'm Done" button at the bottom of the page for your forms to be saved.

DO NOT register via OnPatient directly or through you mobile app as we cannot link your account to our practice after registration from those methods..

Links expires in 7 days after which you will have to contact office to send you a new invitation.

Tip: Access step-by-step instructions for registration. After registration via the email invitation, you should download the mobile app to conveniently access your portal information and messages on your phone.

Obtain historical medical records

In select cases we either require or strongly prefer prior medical records. If a Patient Success Team advised you to request records from a previous healthcare provider, please visit our Records page for further instructions. If you have them available you can upload them directly to On Patient as a message attachment.

Tip: Have your previous provider's office information ready for the ROI form if you would like our assistance in obtaining the records.
