Our patient portal, On Patient, serves as a convenient hub for communication and access to essential resources. Here, you can securely update your account information such as demographics, insurance, pharmacy, and payment information. You can also access the portal 24/7 to review appointments and messages, share updates, and access educational materials.

Add upcoming visit to your calendar
Confirm you received a Google Calendar email invite titled “Evolve Psychiatry Telemedicine Consultation” with a secure Google Meet link for your visit. For in office visits, you will attend the appointment address listed in the invite but still need to first access the meeting link to meet with our Patient Navigator before your doctor comes and greets you. If you don’t see the invite in your inbox, be sure to check your spam folder.

Completed intakes are uploaded to patient charts at 5pm every day. Complete these by 5pm the day before your visit. Late submissions may not be accessible by your provider resulting in delays in your treatment plan.
Access the On Patient registration invitation you received at the time of scheduling or the most recent appointment reminder email you received in your inbox. DO NOT register via Onpatient directly as we cannot link your account to our practice after registration. Links expires in 7 days after which you will have to contact office to send you a new invitation.
Sign our consent forms
After logging into On Patient you will be prompted to sign our consent forms. Once signed, these will be saved to your chart and portal so you can review them any time you want. You will verify and complete your account information including the most up-to-date insurance policy, allergies, pharmacy, and payment information.
Tip: Make sure you click "I"m Done" after signing all forms.
Request past records
In select cases we either require or strongly prefer prior medical records. If a Patient Success Team advised you to request records from a previous healthcare provider, please visit our Records page for further instructions.
Tip: Have your previous provider's office information ready for the ROI form if you would like our assistance in obtaining the records.
