Legal Cases

Evolve Psychiatry does not participate in legal cases including any criminal, civil, family, worker’s compensation, and personal injury as well as long-term disability documentation. In order to determine Short Term Disability (STD) and Family and Medical Leave Act (FMLA) eligibility, patients must be established with our practice for at least 3-5 visits. Each individual case is subject to provider discretion. Fees for completing forms are not covered by insurance and are based on a private pay fee schedule listed online.
