Signing up for On Patient
OnPatient is a patient portal developed by our new medical records system, DrChrono EHR, that connects patients and providers. It is an easy, connected, and mobile way to manage your personal health information. OnPatient enables you to schedule appointments, communicate with your healthcare provider, review your health history information, and more - from any online or iOS compatible interface - iPhone or iPad.
Our office has sent you an invitation to the email address provided to us The invitation will expire 7 days after being sent. If your link is expired, you will need to contact the Patient Success Team and request a new invitation email.
1. Click on the Sign Up Now button to begin.
Locate the On Patient invite in your email. Check your spam if you don't see it.
2. Enter the following information and select Sign Up.

Last name
-
Email (must be the same email address used for the invitation)

Create a password
3. Next, validate the account and select Confirm and Connect
Use your Social security number if given to the office --OR-- Date of birth AND phone number
4. Once you have connected, you will see the appointments page. Click Check In.
5. Update your profile.
Provide the most up to date information including contact, insurance, pharmacy, and allergies information.
