SAFETY FIRST
Evolve Mind and Behavioral Wellness offers primarily telemedicine outpatient services. If you feel your patient is in current or imminent crisis, please call 911 or send patient to the nearest emergency room for a crisis psychiatric evaluation.
HOW TO ANSWER
Be sure to answer questions thoroughly. Sparse or incomplete submissions will require a copy of medical records before patient scheduling. Please reference prior records and our practice website for more information.
NEXT STEPS
Our office will call you in the next 2-3 business days for any clarifications or confirmation. For an expedited evaluation, you are welcome to follow-up with our office any time.
