One of the essential requirements for providing optimal care to patients is maintaining accurate and up-to-date patient records. These records provide crucial information about a patient's medical history, previous treatments, and response to medications. By reviewing these records, we can assess the effectiveness and necessity of prescribed medications and treatments, evaluate potential risks or contraindications, and make informed decisions regarding ongoing treatment plans. This process ensures that our patients receive appropriate and safe management of their conditions, minimizing the potential for adverse effects and promoting their overall well-being.

We understand that the process of providing records may appear burdensome or time-consuming for patients. However, it is an integral part of responsible medical practices that enables us to deliver the highest standard of care. Please rest assured that we handle all patient records with strict confidentiality, adhering to applicable privacy laws and regulations.

To ensure a seamless transition of patient care to Evolve Psychiatry, we advise patients to obtain a copy of their medical records from their previous provider(s) and send them to our team in advance of the initial consultation. While records are not required to schedule the initial consultation, they must be provided before treatment planning is initiated. This allows our team to thoroughly review your medical history and develop a personalized treatment plan that best meets your specific needs.

Continuing Stimulants for ADHD

Continuing Sedatives or Hypnotics
Medically Complex or Age > 50
History of Higher Levels of Care
History of Self-Harm
History of Autism or Intellectual Disability
