To ensure a seamless transition of patient care to Evolve Psychiatry, we advise patients to obtain a copy of their medical records from their previous provider(s) and send them to our team in advance of the initial consultation. While records are not required to schedule the initial consultation, they must be provided before treatment planning is initiated.
